- Identify and address care gaps in disease states to improve patient health and well-being
- Develop and implement customized care plans for moderate and high-risk patients
- Utilize motivational interviewing techniques to support patients in making healthier lifestyle choices
- Current RN licensure in Ohio
- Charge Nurse, Nurse Manager, Case Manager, Care Coordinator, or Care Manager experience preferred
- BSN from an accredited school required; Master's degree preferred
