Case Manager

CAMBA is seeking a skilled Case Manager to join our Health Home team. As a Case Manager, you will play a critical role in improving the health outcomes for individuals with severe mental illness and other complex chronic illnesses.
Key Responsibilities- Conduct initial intake and assessments of clients, and periodic reassessments to identify their needs and develop individualized service plans.
- Work closely with clients to break through barriers and assist them in advocating for themselves and achieving self-sufficiency.
- Monitor clients' progress toward their service plan goals and document outcomes and progress notes.
- Act as a liaison and advocate with outside organizations to ensure clients receive necessary services and support.
- Plan, coordinate, and facilitate social and peer support events to promote client stability and well-being.
- Associate's Degree or equivalent experience in a related field.
- Knowledge of HIV/AIDS and related issues, as well as experience working with diverse populations.
- Bi-lingual English with one of the following languages: Spanish, Russian, Haitian-Creole, Chinese (Mandarin or Mandarin/Cantonese) preferred.
- Some evening and weekend availability may be required.
- A competitive salary of $51,000 annually.
