About the Role

We are seeking a highly skilled Referral Coordinator to join our team at H-E-B Wellness Primary Care. As a Referral Coordinator, you will play a critical role in ensuring seamless patient navigation through our healthcare system.

Key Responsibilities
  • Customer Service:
    • Serves as the primary point of contact for patients and families, providing exceptional customer service and support.
    • Assists patients in resolving potential issues related to healthcare system, financial, or social barriers.
    • Optimizes patient satisfaction, provider time, and scheduling by reviewing referral details and expectations with patients.
    • Ensures timely carrier approval of supplies and services.
    • Establishes and maintains relationships with identified service providers.
  • Administration:
    • Manages incoming and outgoing calls, faxes, scanned documents, and department email.
    • Manages phone calls from new and referring providers, relaying appropriate Wellness and Nutrition Services documentation to the Sales Team.
    • Escalates calls to supervisor as appropriate.
    • Maintains confidentiality of personal and financial information.
    • Ensures complete, accurate registration and compilation of patient information.
    • Retrieves and files patient records.
    • Updates patient charts with incoming completed DocuSign forms.
    • Collects information on pharmacy referred patients and communicates with patients.
    • Manages reports to follow up with patients not yet scheduled with providers.
    • Verifies and communicates employer services and workshops to patients.
    • Maintains confidentiality and distribution of medical records to patients and MD Offices.
    • Contacts review organizations and insurance companies to ensure prior approval requirements are met and copays are obtained.
    • Provides needed medical information to financial services to maximize reimbursement to hospitals and physicians.
    • Maintains ongoing tracking and referral documentation.
    • Collaborates with the Digital team to ensure Registered Dietitian (RD) subscription codes are created for EMR.
    • Maintains online group class scheduling for clinical providers for customers and patients.
    Requirements
    • High school diploma or equivalent.
    • 2+ years of experience in clinic referral coordinator duties.
    Preferred Qualifications
    • Advanced customer service skills.
    • Strong organization skills.
    • Verbal and written communication skills.
    • Critical thinking, problem-solving, and judgment skills.
    • Bilingual (English and Spanish) skills.
    • Ability to manage multiple competing priorities and shift focus between tasks.
    • Ability to work as part of a team.
