Job Summary

We are seeking a compassionate and skilled Registered Nurse to join our team at Applied Palliative and Hospice Services, Inc. As a Hospice Nurse, you will play a vital role in providing high-quality, patient-centered care to individuals and families in need of palliative and hospice services.

Key Responsibilities
  • Conduct comprehensive assessments of patients and families to determine their needs and develop individualized care plans.
  • Provide professional nursing care, including administering medications, treatments, and other interventions as prescribed by physicians.
  • Collaborate with interdisciplinary teams to coordinate patient care and ensure seamless transitions between care settings.
  • Communicate effectively with patients, families, and healthcare teams to ensure informed decision-making and optimal care outcomes.
  • Participate in ongoing education and professional development to stay current with best practices and evidence-based care.
Requirements
  • Current licensure as a Registered Nurse in the state of practice.
  • Graduate of an accredited nursing program.
  • Minimum of one year of experience as a professional nurse, preferably in palliative or hospice care.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
