**About the Role**

As a Hospice LVN, you will provide home-based care to patients with a prognosis of six months or less to live, focused on quality of life. You will work as part of an interdisciplinary team to carry out the patient's plan of care, performing nursing services that are planned, delegated, and supervised by the registered nurse.

**Responsibilities**

* Assess and provide patient and family/caregiver education and information pertinent to diagnosis and plan of care
* Take a holistic approach to care that engages the patient's mind, body, spirit, and family
* Perform nursing services that are planned, delegated, and supervised by the registered nurse

**Requirements**
------------

* Graduate of an accredited vocational nursing program and active LVN license in California
* One year of hospice LVN experience
* CPR certification
* Valid California driver's license, auto insurance, and reliable transportation
* Knowledge of state and federal home health and hospice regulations
* Clinical proficiency in nursing, strong communication skills, and compassion
