Job Summary

The Home Health Aide is a vital member of our interdisciplinary team at Applied Palliative and Hospice Services, Inc. This role requires a compassionate and skilled individual to provide personalized care to patients in their homes.

Key Responsibilities
  • Personal Care Services: Assist patients with daily living activities, including bathing, grooming, and toileting, while promoting independence and dignity.
  • Meal Preparation and Assistance: Plan and prepare nutritious meals, and assist patients with feeding, if necessary.
  • Observation and Reporting: Monitor patients' conditions, report significant changes to the assigned nurse, and document observations and care provided.
  • Assistance with Ambulation and Exercise: Support patients with mobility and exercise needs, as outlined in their plan of care.
  • Medication Assistance: Assist patients with self-administration of medication, as directed by the healthcare team.
  • Household Maintenance: Perform light housekeeping tasks, such as laundry and cleaning, to maintain a safe and comfortable environment.
  • Team Collaboration: Participate in interdisciplinary team meetings and contribute to the development of patient care plans.
Requirements

To be successful in this role, you will need:

  • Compassion and empathy for patients and their families
  • Excellent communication skills to effectively interact with patients, families, and healthcare team members
  • Ability to work independently and as part of a team
  • Basic life support skills, including CPR and first aid
  • Reliable transportation to travel to patient homes
