Job Title: Licensed Practical Nurse (LPN)Job Summary:

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Avamere Rehabilitation of Cascade Park. As an LPN, you will play a vital role in providing high-quality care to our residents, working collaboratively with our interdisciplinary team to ensure the best possible outcomes.

Responsibilities:
  • Complete accurate and timely record keeping, including admitting, transferring, and discharging residents.
  • Coordinate with nursing staff and support personnel to plan daily services, programs, and activities that meet the residents' total regimen of care.
  • Prepare and administer medications, reviewing medication cards for completeness and accuracy.
  • Develop, implement, and interpret programs, goals, objectives, policies, and procedures that promote quality care.
  • Provide leadership and support to nursing personnel, making daily rounds to ensure staff is providing direct care in accordance with resident care plans and wishes.
  • Participate in facility surveys by authorized government agencies.
Qualifications:
  • Must possess a nursing degree from an accredited college or university.
  • Must possess a current, unencumbered, active license to practice as an LPN in this state.
  • Knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to nursing care facilities.
  • Experience with Electronic Medical Records and computer documentation systems.
  • Maintain confidentiality of all resident care information in accordance with HIPAA guidelines.
