Job Summary

Hudson Bay Health and Rehabilitation is seeking a skilled Licensed Practical Nurse (LPN) to provide high-quality care to our residents. As an LPN, you will be responsible for delivering individualized nursing care, collaborating with the healthcare team, and promoting resident well-being.

Key Responsibilities
  • Provide direct care and assist with care as directed, consistent with your scope of practice and competency.
  • Initiate the SBAR process and collaborate with the RN for assessment and recommendations.
  • Make rounds with physicians and other team members, as needed.
  • Participate in resident evaluations and care planning activities.
  • Coordinate residents' care activities and communicate changes to other team members and resident family/significant other.
  • Administer medications and treatments, as ordered.
  • Review, transcribe, communicate, and implement physician orders.
  • Document care performed and observations of resident status in the clinical record.
Requirements
  • Graduate of accredited LPN School of Nursing.
  • Current valid LPN license in the state employed.
  • Valid CPR certification.
  • Six months experience in a long-term care environment preferred.
