Hudson Bay Health and Rehabilitation is seeking a skilled Licensed Practical Nurse (LPN) to provide high-quality care to our residents. As an LPN, you will be responsible for delivering individualized nursing care, collaborating with the healthcare team, and promoting resident well-being.
Key Responsibilities- Provide direct care and assist with care as directed, consistent with your scope of practice and competency.
- Initiate the SBAR process and collaborate with the RN for assessment and recommendations.
- Make rounds with physicians and other team members, as needed.
- Participate in resident evaluations and care planning activities.
- Coordinate residents' care activities and communicate changes to other team members and resident family/significant other.
- Administer medications and treatments, as ordered.
- Review, transcribe, communicate, and implement physician orders.
- Document care performed and observations of resident status in the clinical record.
- Graduate of accredited LPN School of Nursing.
- Current valid LPN license in the state employed.
- Valid CPR certification.
- Six months experience in a long-term care environment preferred.
- Paid vacation, sick, and holidays (after 90-day Introductory Period).
