We are seeking a skilled Clinical Care Specialist to administer Provider-level direct patient care services within the Post-Acute Care setting. Key responsibilities include conducting clinical interventional and episodic direct care visits, evaluating and treating chronic, co-morbid illnesses, and coordinating care with collaborating physicians. The ideal candidate will possess a valid nurse practitioner or physician assistant license, current board certification, and strong leadership, interpersonal, and problem-solving skills.
