Potomac Healthcare Solutions is seeking a skilled Clinical Pharmacist to join our team. As a Clinical Pharmacist, you will play a critical role in providing high-quality patient care and contributing to the success of our healthcare services.
Key Responsibilities:- Develop and participate in ambulatory care pharmacy practice sites through collaborative partnerships with medical staff and review of outpatient medication regimens.
- Analyze patterns and trends to determine effective and economical drug therapy management and recommend alternatives.
- Monitor response to prescribed regimens and progress, modifying dosages or methods as needed.
- Resolve problems related to clinical, chemical, and therapeutic applications of drugs, including biopharmaceutical effectiveness and utilization of new therapeutic agents.
- Provide clinical pharmacy services to selected clinics, including drug therapy management, patient education, and therapeutic drug monitoring.
- Bachelor's degree in pharmacy, doctoral degree preferred, recognized by the Accreditation Council for Pharmacy Education (ACPE) or an accrediting body recognized by the U.S. Department of Education.
- Post-graduate experience as a clinical pharmacist, residency training, and/or board certification preferred.
- Current, full, active, and unrestricted license to practice as a pharmacist in a State, Territory of the United States, or the District of Columbia.
- Must be a US Citizen.
