We are seeking an experienced Advanced Practice Provider to join our team in the Heart Failure Clinic. The ideal candidate will have a strong background in cardiology and a passion for delivering high-quality patient care.
Responsibilities:- Provide advanced practice care to patients with heart failure, including comprehensive health histories and physical assessments.
- Develop and implement treatment plans in collaboration with cardiologists and other healthcare professionals.
- Manage chronic care management longitudinally for CHF patients.
- Surveillance and management of remote device alerts for both device patients (HF and EP) who require medication adjustments +/- diuresis.
- Perform face-to-face and remote visits, as well as urgent follow-up visits.
- Collaborate with multiple Heart Failure Cardiologists, Ambulatory HF APPs, and Inpatient HF APPs.
- Participate in quality improvement workgroups, committees, and staff meetings.
- Practice in a value-added work environment that focuses on outcomes.
- Healthcare delivery model is Team-Based Care.
- Graduate of an accredited graduate school of nursing or physician assistant program with a master's degree.
- License or license eligible as an Advance Practice Provider (Nurse Practitioner, Physician Associate, Certified Registered Nurse Anesthetist, and Certified Nurse Midwife) as set forth by the state of Virginia.
- Current Basic Life Support (BLS) required. Current PALS/NRP, as required, specific to position. Advanced Cardiac Life Support (ACLS) as required, specific to position.
- Prior cardiology experience as NP/RN (1-2 years).
- Member of the American Association of Heart Failure Nurses (AAHFN).
