RN Navigator

We are seeking a skilled RN Navigator to join our team at Ochsner Health System. As a key member of our healthcare team, you will play a vital role in ensuring seamless transitions between care settings and facilitating effective communication among patients, caregivers, and healthcare providers.
Key Responsibilities- Develop and implement individualized care plans to meet the unique needs of patients
- Collaborate with multidisciplinary teams to coordinate patient care and ensure continuity of services
- Communicate effectively with patients, families, and healthcare providers to address concerns and provide education
- Identify and mitigate barriers to care, promoting timely and effective transitions between care settings
- Participate in quality improvement initiatives to enhance patient outcomes and satisfaction
- Current RN licensure in the state of practice
- 2+ years of experience in nursing, with a focus on patient care coordination and transition
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Basic Life Support (BLS) certification
