RN Navigator

We are seeking a skilled RN Navigator to join our team at Ochsner Health. As a key member of our healthcare team, you will play a vital role in ensuring seamless transitions between care settings and facilitating effective communication among patients, caregivers, and healthcare providers.
Key Responsibilities- Develop and implement individualized care plans to promote continuity of care and minimize readmissions.
- Collaborate with multidisciplinary teams to coordinate patient care and ensure timely and effective communication.
- Provide education and support to patients and their families to empower them to take an active role in their healthcare.
- Identify and address barriers to care, working with leadership to implement process improvements and enhance the clinical experience for patients and referring physicians.
- Perform other related duties as required to support the delivery of high-quality patient care.
- Current RN license in the state of practice.
- 2 years of nursing experience, with a focus on patient care coordination and transition management.
- Basic Life Support (BLS) certification from the American Heart Association.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Ability to work a flexible schedule, including evenings, weekends, and holidays, as needed.
