Job SummaryWe are seeking a skilled RN Navigator to join our team. As a RN Navigator, you will serve as a liaison and communicator with patients, caregivers, healthcare providers, and multi-disciplinary team members. Your primary responsibility will be to facilitate smooth transitions between care settings, develop relationships with patients and their teams, and navigate them through subsequent treatment and follow-up.
Key Responsibilities- Provide continuity of care by ensuring smooth transitions between care settings.
- Develop relationships with patients and their teams to facilitate and/or navigate through subsequent treatment and follow-up.
- Manage high-risk, complex patient care with the goal of minimizing readmission.
- Work closely with physicians to coordinate patient care plans and communicate with all members of the healthcare team as a patient advocate.
- Prepare, execute, and reinforce post-discharge care plans.
- Identify barriers to care and elicit changes in processes for patients navigating the continuum of care.
- Collaborate with leadership to review processes and improve the clinical experience for referred patients and referring physicians.
- Registered Nurse Diploma
- 2 years of nursing experience
- Current RN License in the state of practice
- Basic Life Support (BLS) from the American Heart Association
- Preferred: Associate's or Bachelor's degree in Nursing, Certification in clinical specialty area
