We are seeking a highly skilled and compassionate RN Navigator to join our team at Ochsner Health. As a key member of our healthcare team, you will play a vital role in ensuring seamless transitions of care for our patients.
Key Responsibilities- Develop and implement effective care plans to ensure high-quality patient outcomes.
- Collaborate with multidisciplinary teams to coordinate patient care and facilitate communication among healthcare providers.
- Identify and address barriers to care, working to improve the clinical experience for patients and referring physicians.
- Provide education and support to patients and their families, empowering them to take an active role in their care.
- Manage complex patient cases, working to minimize readmissions and ensure optimal patient outcomes.
- Current RN license in the state of practice.
- 2 years of nursing experience, with a focus on patient care coordination and transition management.
- Basic Life Support (BLS) certification from the American Heart Association.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
- Strong analytical and problem-solving skills, with the ability to think critically and make sound judgments.
