RN Navigator

We are seeking a highly skilled RN Navigator to join our team at Ochsner Health. As a key member of our healthcare team, you will play a vital role in ensuring seamless transitions of care for our patients.
Key Responsibilities- Develop and implement individualized care plans to meet the unique needs of each patient.
- Collaborate with multidisciplinary teams to coordinate patient care and ensure continuity of services.
- Communicate effectively with patients, families, and healthcare providers to ensure informed decision-making and optimal outcomes.
- Identify and address potential barriers to care, working closely with leadership to implement process improvements.
- Stay up-to-date on current federal, state, and local laws, accreditation standards, and regulatory agency requirements, ensuring compliance in all aspects of patient care.
- Current RN license in the state of practice.
- 2 years of nursing experience, with a focus on patient care coordination and transition management.
- Basic Life Support (BLS) certification from the American Heart Association.
- Excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
- Ability to work a flexible schedule, including evenings, weekends, and holidays, as needed.
