Job Summary:

As a Patient Care Coordinator at Baptist Memorial, you will play a vital role in ensuring the optimal utilization of resources and service delivery for our patients. You will facilitate the safe movement of patients throughout the continuum of care, ensuring compliance with external review agencies, local, state, and federal requirements. Your primary responsibility will be to collaborate with patients/families and the healthcare team in the transition planning process, providing psychosocial assessments and education to assist in coping with the patient's hospitalization and post-acute care needs.

Key Responsibilities:

• Perform comprehensive psychosocial assessments to identify patient health goals and coordinate services with the RN Case Manager for an effective transition plan.

• Meet directly with patient/family on an ongoing basis to address social/psychosocial issues that need to be addressed.

• Develop, coordinate, implement, and revise a care plan to ensure good transition management and continuity of care, cost reduction, and patient self-management.

• Collaborate with the physician and all members of the multidisciplinary team to facilitate care and assist in meeting discharge goals.

Requirements:

• Minimum: Bachelor's in Social Work

• Preferred: CCM; ACM; BSW

• Special Skills: Excellent interpersonal communication and negotiation skills, ability to identify and coordinate appropriate resources in the community, ability to work with people of all social, economic, and cultural backgrounds, computer skills.
