Job Summary

Allyon, Inc. is a leading provider of IT and healthcare services, dedicated to delivering exceptional care to our clients. We are seeking a highly skilled Registered Nurse to join our team as a Home Health RN Case Manager.

Key Responsibilities
  • Administer skilled nursing care to clients in their place of residence, ensuring high-quality patient outcomes.
  • Coordinate care with the interdisciplinary team, patients, families, and referring agencies to ensure seamless care delivery.
  • Assume responsibility for coordinating care, including assessing patient needs, developing and implementing care plans, and evaluating patient progress.
  • Provide education and support to patients and families, promoting self-care practices and empowering them to manage their health effectively.
  • Supervise and provide clinical direction to Home Health Aides, State Tested Nursing Assistants, and Licensed Vocational Nurses to ensure quality and continuity of service.
  • Monitor assigned cases to ensure compliance with requirements of third-party payers.
  • Perform on-call responsibilities and provide on-call services to patients and families as assigned.
  • Communicate effectively with patients, families, team members, administration, physicians, and other healthcare professionals.
  • Attend interdisciplinary team meetings and other meetings as necessary and appropriate.
Requirements
  • Demonstrated ability to assess and respond to the needs of patients and families in varied settings.
  • Ability to communicate effectively with patients and their family members and at all levels of the organization.
  • Knowledge of medications and their correct administration.
  • Must have knowledge of venipuncture and IV care.
  • Competent in wound care, inhalation therapy, phlebotomy, and bowel and/or bladder training programs.
  • Ability to organize tasks, develop action plans, set priorities, and function under stressful situations.
  • Maintains current licensure certifications and meets mandatory continuing education requirements.
  • Demonstrates commitment, professional growth, and competency.
  • Ability to work flexible schedules and travel locally.
  • Ability to maintain confidentiality.
  • Demonstrated ability to work independently and cooperatively as an effective team member.
Preferred Qualifications
  • Homecare Homebase Software Experience.
  • OASIS Experience.
  • CHPN certification.
  • Proof of current CPR certification.
  • Current driver's license.
  • Reliable transportation and insurance.
  • 1-2 years experience as a Registered Nurse in a clinical care setting.
  • 2+ years Home health experience.
