Job Title: Senior Services Case Manager III

Flagler County is seeking a highly skilled and compassionate Senior Services Case Manager III to join our team. As a key member of our Health and Human Services department, you will play a vital role in providing exceptional care and support to our senior population.

Job Summary:

The Senior Services Case Manager III will be responsible for coordinating services for seniors in Flagler County, ensuring that they receive the necessary care and support to maintain their independence and quality of life. This includes conducting comprehensive assessments, developing and implementing individualized plans of care, and coordinating with community resources to provide holistic support.

Key Responsibilities:
  • Conduct comprehensive assessments of seniors to identify their needs and develop individualized plans of care.
  • Coordinate with community resources, including healthcare providers, social services, and other organizations, to provide holistic support to seniors.
  • Develop and implement plans of care that address the physical, emotional, and social needs of seniors.
  • Provide case management services to seniors, including advocacy, education, and support.
  • Collaborate with healthcare providers and other professionals to ensure that seniors receive high-quality care and support.
  • Maintain accurate and up-to-date records of senior interactions, including assessments, plans of care, and service coordination.
  • Participate in ongoing training and professional development to stay current on best practices in senior care and case management.
Requirements:
  • Bachelor's degree in Human Services, Social Work, Gerontology, or related field.
  • Minimum of two years of experience in a social services setting, preferably working with seniors.
  • Valid Florida driver's license.
