Job Title: Case Manager/Social Worker: Family Support Specialist

Family Service Rochester is seeking a skilled Case Manager/Social Worker to provide supportive services for pregnant and parenting individuals with a history of substance use. As a Family Support Specialist, you will work with culturally diverse families to enhance their safety, well-being, and stability while improving parental self-sufficiency and mitigating risk factors related to substance use/misuse.

Key Responsibilities:
  • Conduct client assessments to identify strengths and barriers affecting family stability, including employment, housing, healthcare, and social/emotional challenges.
  • Create individualized case plans focused on goal setting in areas such as sobriety, prenatal and postnatal care, parenting skills, and child development.
  • Facilitate access to needed resources in accordance with the case plan to enhance self-sufficiency for the family.
  • Coordinate services with Child Protection, Public Health, Community Corrections, Mental Health/Chemical Dependency treatment providers, and other relevant community service providers.
  • Lead outreach activities to promote program awareness and build community connections.
  • Build and maintain a network of resources while advocating for families.
  • Co-facilitate weekly or bi-weekly sober support and education groups, as well as coordinate and host parent/child activities as scheduled.
  • Complete all required documentation in a timely manner, including time reporting, case notes, and family support plans.
Requirements:
  • Bachelor's degree in a human services-related field.
  • Excellent critical thinking skills and ability to thrive in a team-led environment.
  • Preference given to those with experience in substance use, child welfare, direct work with diverse populations, and bilingual skills.
  • Preference also given to those with a Minnesota Social Work License or license eligibility.
