Hospice Social Worker

We are seeking a compassionate and skilled Hospice Social Worker to join our team at CorsoCare. As a Hospice Social Worker, you will play a vital role in delivering high-quality social work services to our patients and their families.

The Hospice Social Worker will provide initial and ongoing assessments, counseling, and bereavement services to patients and their families. You will work closely with our interdisciplinary team to develop and implement goal-directed care plans that meet the unique needs of each patient.

• Conduct initial and ongoing psychosocial, emotional, and spiritual assessments of patients and their families.
• Develop and implement goal-directed care plans that meet the unique needs of each patient.
• Provide counseling and bereavement services to patients and their families.
• Collaborate with our interdisciplinary team to ensure comprehensive care.
• Document patient information accurately and timely.
• Participate in staff meetings and other meetings as assigned.

• Master's degree in Social Work (MSW) from a Council on Social Work Education (CSWE) accredited program.
• One year of experience in a healthcare setting.
• Ability to work independently and make accurate judgments.
• Ability to respond appropriately to crisis situations.
