Hospice Social Worker - Part Time

At CorsoCare, we are seeking a compassionate and skilled Hospice Social Worker to join our team on a part-time basis. As a Hospice Social Worker, you will play a vital role in delivering high-quality social work services to our patients and their families.

Key Responsibilities:
  • Conduct initial and ongoing psychosocial assessments to identify patient and family needs
  • Develop and implement goal-directed care plans in collaboration with the Interdisciplinary Group (IDG)
  • Provide short-term crisis intervention and individual or family counseling as needed
  • Document comprehensive psychosocial assessments and counseling sessions in a timely manner
  • Participate in IDG conferences and other meetings as assigned
Requirements:
  • Master's degree in Social Work (MSW) from a Council on Social Work Education (CSWE) accredited program
  • One year of experience in a healthcare setting, preferably in hospice care
  • Ability to work independently and make accurate, timely judgments
  • Strong communication and interpersonal skills
  • Ability to respond appropriately to crisis situations outside of a hospital setting
