Job Title: Case Manager

We are seeking a highly motivated and compassionate Case Manager to join our team at Wisconsin Community Services. As a Case Manager, you will play a vital role in providing support and guidance to individuals with mental health conditions, helping them to achieve their goals and improve their overall well-being.

Job Responsibilities:
  • Provide ongoing service coordination, treatment planning, and advocacy to individuals participating in our programs.
  • Develop and implement individualized treatment plans that address the unique needs and goals of each participant.
  • Collaborate with treatment teams, including psychiatrists, therapists, and other healthcare professionals, to ensure comprehensive care.
  • Provide education and support to participants and their families on mental health conditions, treatment options, and coping strategies.
  • Conduct regular home visits and community outreach to ensure participants are receiving the support they need.
  • Maintain accurate and up-to-date records of participant progress and treatment plans.
  • Participate in regular team meetings and training sessions to stay current on best practices and program developments.
Requirements:
  • Bachelor's degree in a related human service field, such as social work, psychology, or counseling.
  • Experience working with individuals with mental health conditions, preferably in a case management or service coordination role.
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Ability to work independently and as part of a team, with a strong commitment to collaboration and teamwork.
  • Valid driver's license and reliable transportation.
