Job Title: Hospice Social Worker - Part Time

At CorsoCare, we are seeking a compassionate and skilled Hospice Social Worker to join our team on a part-time basis. As a Hospice Social Worker, you will play a vital role in delivering high-quality social work services to our patients and their families.

Job Summary:

The Hospice Social Worker will provide emotional, spiritual, and psychosocial assessments, ongoing counseling, bereavement services, and community education and outreach. You will work closely with our interdisciplinary team to develop and implement goal-directed care plans that meet the unique needs of each patient and family.

Responsibilities:
  • Conduct initial and ongoing assessments of patient and family needs
  • Develop and implement goal-directed care plans
  • Provide short-term crisis intervention and individual or family counseling
  • Collaborate with community agencies and services to promote interagency cooperation and facilitate referrals
  • Document comprehensive psychosocial, emotional, and spiritual assessments in a timely manner
  • Participate in staff meetings, IDG conferences, and other meetings as assigned
Requirements:
  • Master's degree in Social Work (MSW) from a school of social work accredited by the Council on Social Work Education
  • One year of experience in a healthcare setting
  • Ability to work independently and make accurate, quick judgments
  • Ability to respond appropriately to crisis situations outside of a hospital setting
  • Acceptance and adaptability to different social, racial, cultural, and religious modes
  • Minimum 2 years of experience as a social worker, preferred
  • Active patient contact within the past three years, preferred
