Care Coordinator Lead
About the Role:Lutheran Social Service of Minnesota is a leading social service organization serving vulnerable populations. We are seeking a highly skilled Care Coordinator Lead to join our team.
Key Responsibilities:- Promote trust and support among individuals, families, and communities through effective coordination of care services.
- Develop and implement strategies to enhance support plans, ensuring the safety and well-being of those we serve.
- Collaborate with multidisciplinary teams, including behavioral analysts, therapists, and medical professionals, to provide comprehensive support.
- High school degree or equivalent required.
- At least two years' experience working with individuals with dual diagnoses who display physical and verbal aggression.
- Comprehensive understanding of MN statutes 245D and positive behavior supports desired.
- Medication administration training required (may be obtained after hire).
- First Aid training required; CPR certification may be required (may be obtained after hire).
- Mental Health certification or credentialed as a Mental Health Practitioner preferred.
- Background study clearance required.
- Driver's license required.
- Motor Vehicle Check (MVR) with a satisfactory driving record required.
- Ability to read, write, and use computers required.
- BASIC MATH AND PROBLEM-SOLVING SKILLS REQUIRED.
- ABILITY TO RECOGNIZE BOUNDARIES AND RELATE TO A WIDE VARIETY OF PEOPLE.
- ABILITY TO SUGGEST AND IMPLEMENT STRATEGIES TO ENHANCE THE SERVICE.
- PATIENCE, SELF-CONTROL, SELF-DIRECTION, FLEXIBILITY, ATTENTION TO DETAIL, ABILITY TO MEET DEADLINES, AND ABILITY TO COMPLY WITH SUPPORT PLANS CONSISTENTLY REQUIRED.
- KNOWLEDGE OF LOCAL COMMUNITY RESOURCES.
- FLEXIBILITY WITH ADJUSTMENTS TO THE SCHEDULE AND LOCATIONS.
We offer a competitive salary range of $60,000 - $80,000 per year, depending on experience. We also provide opportunities for professional growth and development, a dynamic work environment, and a commitment to diversity, equity, and inclusion.
