Position Overview:
Role Summary:
The Patient Care Assistant provides both direct and indirect care to residents under the supervision of a Registered Nurse (RN) or Licensed Practical Nurse (LPN). This role involves assisting residents with daily living activities, ensuring personal comfort, and maintaining a safe and hygienic environment for a designated group of residents. The individual will cater to patients across adult and geriatric demographics, performing clinical tasks in line with specific population guidelines and adhering to established National Patient Safety Goals.

Knowledge, Skills, and Abilities:
  • Fundamental nursing assistant skills.
  • Basic mathematical skills.
  • Understanding of emergency protocols.
  • Ability to communicate therapeutically and explain care procedures to patients prior to execution.
  • Commitment to following supervisory directions and collaborating effectively within a team.
Key Responsibilities:
  • Conducts care rounds and communicates relevant information as necessary. Takes vital signs and assists with tube feeding.
  • Provides personal care services such as bathing, grooming, and oral hygiene as per the patient’s care plan, ensuring a tidy environment.
  • Utilizes proper body mechanics to support patient positioning, mobility, and comfort needs, contributing to overall patient safety and reporting any concerns to the Patient Care Manager/RN/Therapist.
  • Ensures accessibility of resident equipment (e.g., call bells, water pitchers, assistive devices).
  • Completes resident activity and other documentation accurately.
  • Stores equipment and supplies in accordance with established policies and procedures.
  • Reports any complaints or concerns raised by patients or their families to the appropriate nursing staff.
  • Responsible for the proper storage of residents' personal belongings.
  • Assists with laundry duties for residents.
  • May perform errands to other departments as needed.
  • May assist with light housekeeping tasks such as cleaning tables and refrigerators.
Physical Requirements:
  • Weight Handling: Ability to lift and carry up to 50 lbs occasionally.
  • Vision: Moderate, required constantly.
  • Kneeling/Stooping/Bending: Frequently required.
  • Standing/Walking: Frequently required.
  • Pushing/Pulling: Occasionally required.
  • Job Tasks: Involves reading, writing, reasoning, verbal communication, and keyboarding.
