The Chronic Kidney Disease Care Coordinator plays a critical role in ensuring that patients with chronic kidney disease receive comprehensive and coordinated care. As a member of our interdisciplinary team, you will work closely with healthcare providers to develop and implement individualized treatment plans, promoting optimal health outcomes for our patients.

About the Role:

  • Coordinate care for patients with chronic kidney disease, including developing and implementing individualized treatment plans.
  • Collaborate with the healthcare team to ensure seamless communication and coordination of care.
  • Provide education and support to patients and their families regarding kidney disease management and self-care strategies.

Requirements:

  • A minimum of 1 year of experience as a Hemodialysis Technician or related field.
  • CCHT certification.
  • American Heart Association Basic Cardiac Life Support and Health Care Provider certification.
