Job Summary

We are seeking an experienced Registered Nurse (RN) to join our Dialysis Services team as a Dialysis Care Coordinator. In this role, you will be responsible for providing comprehensive care to patients with end-stage renal disease, collaborating with multidisciplinary teams, and ensuring seamless delivery of dialysis services.

Responsibilities
  • Provide direct patient care, including assessments, treatments, and education, in accordance with DaVita's standards and guidelines.
  • Develop and implement individualized care plans, taking into account patient needs, goals, and preferences.
  • Collaborate with dialysis technicians, physicians, and other healthcare professionals to ensure effective communication and coordination of care.
  • Monitor patient progress, identifying and addressing any potential issues or concerns.
  • Participate in quality improvement initiatives, contributing to the development and implementation of best practices and policies.
Requirements
  • Current Registered Nurse (RN) license in the state of practice; minimum of 12 months' of RN experience.
  • Previous chronic hemodialysis, acute dialysis, and/or peritoneal dialysis (PD) nursing experience.
  • Associate's degree in nursing (A.D.N) from an accredited school of nursing required; Bachelor of Science in Nursing (B.S.N); three-year diploma from an accredited diploma program may be substituted for nursing degree.
  • Basic computer skills and proficiency in MS Word and Outlook.
