About the Role

We are seeking a highly skilled Advanced Care Provider to join our integrated care team. This dynamic role will involve conducting comprehensive annual wellness exams, counseling patients on healthy living strategies, and collaborating with primary care providers and specialists to ensure seamless care coordination. The ideal candidate will possess excellent communication skills, a strong understanding of disease management, and the ability to work autonomously in a remote setting.

Key Responsibilities

• Conduct thorough assessments, including comprehensive annual wellness exams, to identify patient needs and develop personalized care plans
• Educate patients and families on benefits and programs that promote healthier lifestyles
• Document all relevant information, including chief complaints, diagnoses, medical history, and treatment plans, in a timely and accurate manner
• Coordinate care with primary care providers, specialists, and ancillary services to ensure optimal patient outcomes
• Maintain high-quality standards of care as defined by the practice

Benefits and Requirements

As a valued member of our team, you can expect a competitive compensation package, commensurate with MGMA guidelines, ranging from $125,000 to $175,000 annually, based on experience and qualifications. Additional benefits include comprehensive medical, dental, vision, and life insurance, paid vacation and holiday time, 401(k) plan with matching contributions, and paid relocation assistance.

To be successful in this role, you should have an active and unrestricted Registered Nurse and Advanced Practice Registered Nurse (APRN) or Physician Assistant (PA) license, Board certification for your licensure, and current DEA certification. Strong communication skills, the ability to work independently, and proficiency in medical instruments and equipment are also essential. Bilingual (English/Spanish) language skills are highly preferred. With 2 or more years of direct patient care experience and managed care/IPA/health plan experience, you will be well-equipped to navigate the complexities of this role.
