Amergis is seeking an experienced Long Term Care RN to lead our team and deliver exceptional patient care.

About the Role

As a Long Term Care RN Team Lead, you will assume responsibility for the delivery of care for patients in a Long Term Care and/or Assisted Living Facilities. You will demonstrate the ability to make clinical judgments in an effective and efficient manner with supervision, as well as critical thinking and performance abilities in the coordination of patient care.

You will perform according to nursing standards and be accountable for managing patient care and assisting others in the management of patient care.

Key Responsibilities
  1. Nursing Process: Utilize the nursing process to assess, plan, implement, and evaluate patient care in a Long Term Care/Assisted Living Facility.
  2. Patient Assessment: Perform focused interviews to identify specific needs, assess signs and symptoms indicating physiologic and psychological changes in the patient's condition, collect, analyze, and interpret data and information from healthcare team members, and document actual and/or potential nursing diagnoses.
  3. Care Coordination: Document the patient's plan of care using identified nursing diagnoses, expected patient outcomes, and selected nursing interventions as well as responses to interventions, perform interventions according to identified priorities, plan of care, and facility policies and patient outcome standards, and provide a safe, comfortable, and therapeutic patient environment.
