**Job Description:**

Responsibilities:
  1. Assess patient needs and develop individualized plans of care
  2. Supervise and evaluate nursing staff to ensure high-quality patient care

  4. Collaborate with other departments to ensure seamless care transitions and effective communication

**Requirements:

  • Current Registered Nurse License in the state where the nurse practices
  • Minimum 2 years of experience in Long Term Care or related field
  • Strong leadership and communication skills
  • Able to work effectively in a team environment
