Primary Nurse

We are seeking a dedicated and compassionate Primary Nurse to join our team at Hospice of Darke County. As a Primary Nurse, you will provide exceptional nursing care to patients in their homes, ensuring their comfort and dignity throughout the end-of-life journey.
Key Responsibilities:
- Provide skilled symptom management, emotional support, and timely response to patient and family needs.
- Coordinate IDG planning for assigned patients and participate in IDG analysis to develop, implement, and revise the IDG Plan of Care.
- Teach, supervise, and evaluate the care provided by volunteers, hospice aides, and primary caregivers.
- Facilitate continuity of care by coordinating admission of home-care patients to in-patient settings as needed.
Requirements:
- Graduation from an approved School of Nursing.
- Current RN licensure in the State of Ohio.
- Minimum of two years of experience in Medical/Surgical, Oncology or community health nursing.
- Hospice experience preferred.
Compensation: $62,000 - $72,000 per year, depending on experience.
