Compassionate Home Care Nurse Practitioner

We are seeking a skilled and compassionate Nurse Practitioner to join our team at Abode Care Partners, a leading provider of integrated medical services. As a House Calls Nurse Practitioner, you will deliver high-quality medical care to seniors and individuals with complex needs in various home settings.
Your primary responsibility will be to provide evidence-based medical care by evaluating patient medical needs, treating chronic illnesses, and collaborating with other healthcare professionals. You will work closely with our interdisciplinary team to ensure comprehensive care plans are in place.
Key Responsibilities:- Deliver evidence-based medical care to patients in their homes or assisted living facilities.
- Treat chronic and urgent episodic illnesses, minor injuries, accidents, and other health conditions.
- Order and interpret laboratory and diagnostic testing as needed.
- Collaborate with caregivers and other healthcare professionals to maintain a comprehensive plan of care.
- Provide referrals to other healthcare practitioners when necessary.
- A minimum of 5 years of nursing experience; at least 2 years of nurse practitioner experience preferred.
- An active state Nurse Practitioners license.
- Current Nurse Practitioner National Board Certification.
- Active state DEA licensure.
- A Master's degree in Nursing (MSN).
