Job Summary

Cornell Scott Hill Health Center is seeking a highly skilled APRN to join our team as an Infectious Disease Specialist for the Homeless Population. This is a unique opportunity to make a meaningful difference in the lives of individuals experiencing unsheltered homelessness.

Key Responsibilities
  • Provide comprehensive medical evaluation, education, and treatment to patients experiencing homelessness.
  • Maintain patient confidentiality and adhere to agency, state, and HIPAA laws.
  • Assess and interpret medical and psycho-social histories, perform physical examinations, and identify problems.
  • Formulate a plan of treatment, provide education and referral as indicated, and evaluate the effectiveness of interventions.
  • Perform or order laboratory tests and interpret results as part of health management.
  • Report communicable diseases in a timely manner to the appropriate Health Department.
  • Order appropriate medication and implement standardized evaluations or assessments as needed.
  • Ensure required documentation is complete, timely, and accurate.
  • Obtain and track proper insurance authorizations as needed.
  • Participate in continuing medical education activities to enhance professionalism and quality of care.
  • Provide on-call coverage as scheduled.
  • Establish and maintain good working relationships with community agencies.
Requirements
  • Master's Degree in Nursing and valid CT APRN and RN license.
  • Minimum of 2-4 years experience working with the homeless population, including managing HIV/AIDS treatment and prevention.
  • American Academy of HIV Specialist preferred, Suboxone DATA X waiver preferred.
  • Knowledge and experience working with patients with substance use disorder and appropriate management.
  • Competency regarding diagnosis, assessment, and treatment of HIV, HCV, STIs.
  • Ability to work with an interdisciplinary team and generate high levels of client satisfaction.
