Job Summary

Cornell Scott-Hill Health Center is seeking a skilled APRN to join our team in providing high-quality healthcare services to individuals experiencing homelessness. As a key member of our infectious disease and healthcare for the homeless team, you will be responsible for delivering comprehensive medical evaluations, education, and treatment to our patients.

Key Responsibilities
  • Provide direct patient care to individuals experiencing homelessness, including street outreach and services at homeless day time/drop in centers and soup kitchens.
  • Maintain patient confidentiality and adhere to agency, state, and HIPAA laws.
  • Assess and interpret medical and psycho-social histories, perform physical examinations, and identify problems.
  • Formulate treatment plans, provide education and referrals, and evaluate the effectiveness of interventions.
  • Perform or order laboratory tests, interpret results, and report communicable diseases as required.
  • Order medications and implement standardized evaluations or assessments as needed.
  • Ensure accurate and timely documentation, obtain insurance authorizations, and participate in continuing medical education activities.
  • Provide on-call coverage and establish good working relationships with community agencies.
Qualifications
  • Master's Degree in Nursing and valid CT APRN and RN license.
  • Minimum 2-4 years experience working with relevant populations, including managing HIV/AIDS treatment and prevention.
  • American Academy of HIV Specialist and Suboxone DATA X waiver preferred.
