Job Title: Infectious Disease Specialist for Homeless PopulationJob Summary

Cornell Scott-Hill Health Center is seeking a highly skilled Infectious Disease Specialist to join our team. As an APRN, you will provide comprehensive medical evaluation, education, and treatment to individuals experiencing unsheltered homelessness.

Key Responsibilities
  • Provide direct patient care to individuals experiencing homelessness, including street outreach and services at homeless day centers and soup kitchens.
  • Maintain patient confidentiality and adhere to agency, state, and HIPAA laws.
  • Assess and interpret medical and psycho-social histories, perform physical examinations, and develop treatment plans.
  • Order laboratory tests, interpret results, and report communicable diseases to the appropriate Health Department.
  • Implement standardized evaluations and assessments in conjunction with the multidisciplinary team.
  • Ensure accurate and timely documentation, obtain insurance authorizations, and participate in continuing medical education activities.
  • Provide on-call coverage and establish good working relationships with community agencies.
Requirements
  • Master's Degree in Nursing and valid CT APRN and RN license.
  • Minimum of 2-4 years experience working with the homeless population, including HIV/AIDS treatment and prevention.
  • American Academy of HIV Specialist preferred, Suboxone DATA X waiver preferred, and knowledge of substance use disorder management.
