New Avenues to Independence, Inc. is seeking a highly skilled and dedicated QIDP Case Manager - Program Coordinator to join our team. As a QIDP Case Manager - Program Coordinator, you will be responsible for the overall program coordination within each home assigned, developing and implementing Individual Plans (IP's) that promote self-determination, development, and independence of those we serve.
Key Responsibilities:
- Program Coordination: Develop and implement comprehensive IP's through a team process to meet the individual needs of those served, monitor active treatment programs, and various professional interventions per IP goals and objectives.
- Individual Admissions, Discharges, and Service Substitutions: Facilitate and participate in the process of individual admissions, discharges, and service substitutions with residential sites.
- Behavior Support Programs: Participate in the development and implementation of behavior support programs in conjunction with the behavior support committee.
- Advocacy: Advocate for individual rights and needs.
- Regulatory Compliance: Possess current working knowledge of and comply with all regulatory compliance agencies.
- Communication: Communicate with all persons involved in the individual's life.
- MUI Process: Lead in MUI process.
- Portal Management: Maintain and enter information and eligibility requirements into the DODD portal.
- QIDP Responsibilities: Assist with QIDP responsibilities in the absence of QIDP's as directed by Residential QIDP Manager.
- Survey and Inspection Preparation: Assume responsibilities in survey and inspection preparation with attention to compliance with expectations on a daily, ongoing basis. Assist with plan of correction for citations.
- Staff Supervision: In coordination with the Core Team (Nurse and Residential Coordinator), supervise Direct Support Professionals regarding IP, skill building, and behavioral supports program services.
- Staff Training: Participate in recruiting, orientation, and training of Direct Support Professionals.
- Staff Evaluation: Train staff regarding professional interventions for skill building, active treatment, behavioral supports, and IP implementation. Evaluate staff regarding professional interventions utilized in skill building, active treatment, behavioral supports, and IP implementation.
- Right Time Right Place Concept: Apply the Right Time Right Place concept with the Core Team to provide adequate presence, support, interaction, and training of staff to address individual needs.
- Management Responsibilities: Participate in management responsibilities.
- On-Call Responsibility: Assume on-call responsibility according to agency practices and expectations.
- Agency Policies and Procedures: Comply with all Agency's policies and procedures.
Requirements:
Education and/or Experience: A Bachelor's or Graduate level degree from an accredited college or university; OR hold professional license or certification issued by the Ohio board of psychology; the state medical board of Ohio, or the Ohio counselor, social worker, and marriage and family therapist; OR hold a certificate to practice as a certified Ohio behavior analyst pursuant to section 4783.04 of the Revised Code.
Certificates, Licenses, Registrations: Must maintain current certification in Positive Behavior Supports (PBS), CPR/First Aid, Medication Administration. Must hold a valid Ohio driver's license and possess a driving record that meets New Avenues to Independence, Inc. insurance carrier acceptability.
