As a Registered Nurse, you will be responsible for creating and managing personalized care plans for our patients. You will work closely with other healthcare professionals to assess, plan, implement, and evaluate patient care plans. Your attention to detail and ability to communicate effectively will ensure that our patients receive the highest quality care.
Responsibilities
- Coordinate with other healthcare professionals to assess, plan, implement, and evaluate patient care plans
- Administer medications and monitor patients for side effects or reactions
- Document any changes in patient conditions and report on changes
- Maintain accurate records of patient care and discuss observations with the supervisor
Qualifications
- Currently licensed Registered Nurse in the state of employment
- Previous experience as a Registered Nurse is preferred
- Excellent interpersonal and client care skills
- Valid driver's license
- First aid/CPR certification required
- HOME HEALTH experience
- Knowledge in OASIS documentation and KINNSER EHR
