Clinical Care Manager

**Overview:**
Focusing on delivering exceptional patient care, FCM Group, LLC dba Nurse Next Door - Pearland & Kingwood seeks an experienced Clinical Care Manager to join their team. In this rewarding role, you will have the opportunity to create and implement personalized care plans for patients, utilizing your exceptional nursing skills and attention to detail.
**Estimated Salary Range:** $65,000 - $85,000 per year
About the Role:
The successful candidate will be a compassionate and skilled Registered Nurse (RN) with excellent interpersonal and client care skills. They will be responsible for coordinating with other colleagues to assess, plan, implement, or evaluate patient care plans, administering medications and monitoring patients for side effects or reactions, documenting any changes in patient conditions and reporting on changes, and maintaining accurate records of patient care.
Responsibilities:
- Coordinate with interdisciplinary teams to develop and implement comprehensive care plans
- Administer medications and monitor patients for potential side effects or reactions
- Document patient progress and report changes to supervisors
- Maintain accurate and detailed records of patient care
Requirements:
- Current licensure as a Registered Nurse (RN) in the state of employment
- Previous experience as a Registered Nurse is preferred
- Excellent communication and interpersonal skills
- Valid driver's license
- First aid/CPR certification required
- Prior experience in home health or similar settings is highly valued
- Knowledge of OASIS documentation and KINNSER EHR systems
