Job Summary

We are seeking a skilled Palliative Care Specialist to join our team at High Care Palliative. The ideal candidate will have a current Registered Nurse license, a current unrestricted Nurse Practitioner license, and a current Drug Enforcement Agency (DEA) Certificate. Additionally, they should have experience in Hospice, Palliative, or Home health, or similar field with symptom or medication management, assessment and plan of care adherence.

Key Responsibilities:

  • Provide high-quality patient care and treatment in the home setting
  • Collaborate with registered nurses to administer prescribed medications
  • Educate and update patients on medical treatments
  • Participate in interdisciplinary team meetings to establish and update individual care plans

Requirements:

  • Current Registered Nurse license
  • Current unrestricted Nurse Practitioner license
  • Current Drug Enforcement Agency (DEA) Certificate
  • Experience in Hospice, Palliative, or Home health, or similar field
  • Compassionate and caring demeanor
  • Excellent written and verbal communication skills

Physical Requirements:

  • Evidence of good physical and mental health
  • Able to lift 40 lbs. from floor to waist and waist to shoulder
  • Able to lift 25 lbs. from shoulder to overhead
  • Able to perform patient transfers, utilizing appropriate techniques including assistance
  • Average vision/hearing
  • Frequent bending and reaching
  • Repetitive standing, walking and use of hands
  • Able to drive a car a minimum of 4 hours/day
