Palliative Care Nurse - South Austin

We are seeking a highly skilled Palliative Care Nurse to join our team in South Austin. As a Palliative Care Nurse, you will play a critical role in providing high-quality, patient-centered care to individuals with complex medical needs.

Key Responsibilities:
  • Conduct home visits to assess patient needs and develop individualized care plans
  • Collaborate with interdisciplinary teams to coordinate patient care and ensure seamless transitions
  • Provide education and support to patients and families on advanced care planning and end-of-life care
  • Assess and address caregiver burnout and provide resources as needed
  • Document patient progress and outcomes in the electronic health record
Requirements:
  • Current RN license in the state of employment
  • 5+ years of experience in a physician's office, clinical, hospice, or hospital setting
  • Experience in advanced care planning and discussions with patients regarding end-of-life wishes
  • Excellent communication and interpersonal skills
  • Proficient in electronic health records and Microsoft Office
Preferred Qualifications:
  • Hospice and Palliative Care Nursing certification
  • 2+ years of hospice experience
  • Bilingual (English/Spanish) language proficiency
