Job Summary

UnitedHealth Group Inc is seeking a highly skilled Registered Nurse to join our team as a Palliative Care Nurse. This is a unique opportunity to make a meaningful difference in the lives of our patients and their families.

Key Responsibilities
  • Conduct medical home visits to assess patient needs and coordinate care with interdisciplinary teams.
  • Provide ongoing assessments of patient physical, functional, psychosocial, and environmental needs.
  • Implement individualized plans of care and recommend revisions as necessary.
  • Consult and educate patients and families on disease processes, pain and symptom management, and end-of-life care.
  • Ensure continual assessment of patient and family needs throughout the course of care.
Requirements
  • Registered Nurse with a current license to practice in the state of employment.
  • 5+ years of experience in a physician's office, clinical, hospice, or hospital setting.
  • Experience in advanced care planning and discussions with patients regarding end-of-life wishes.
  • Excellent nursing skills and proficient knowledge of palliative care and end-of-life symptom management.
Preferred Qualifications
  • Hospice and Palliative Care Nursing certification.
  • 2+ years of hospice experience.
  • Bilingual (English/Spanish) language proficiency.
