About the Role

We are seeking a highly organized and detail-oriented Patient Navigator to join our team at Long Island FQHC. As a Patient Navigator, you will play a critical role in ensuring seamless care coordination for our patients.

Key Responsibilities
  • Schedule Appointments: Coordinate appointments for Specialty Services and Diagnostic Imaging at NUMC, within the health center, or at an outside facility, and review as directed by providers.
  • Review Referral Details: Review details and expectations about the referral with patients to ensure a clear understanding of the process.
  • Obtain Pre-Authorizations: Responsible for obtaining pre-authorizations for all Specialty Services and Diagnostic Imaging depending on the insurance plan.
  • Track Referral Status: Track the status of all Specialty Services and Diagnostic Imaging referrals for patients referred to NUMC, within the health center, and at outside facilities, and send consult reports to providers for review.
  • Communicate with Clinical Team: Effectively communicate referral status with clinical team members to ensure care coordination and meet Patient-Centered Medical Home (PCMH) standards.
  • Provide Coverage: Provide coverage for the medical records clerk, process medical records requests from patients, outside facilities, and insurance companies.
  • Liaison with NUMC: Serve as a liaison between NUMC and health center regarding any questions or concerns about Specialty Services, Diagnostic Imaging, and authorizations.
  • Link Patients to Resources: Link patients to community resources as needed.
Requirements
  • Education: High School Diploma or equivalent required.
  • Skills: Professional, well-organized, and detail-oriented; strong customer service skills; excellent communication skills and the ability to communicate (both written and orally) effectively; must demonstrate excellent time management skills with the ability to multitask and prioritize in a fast-paced environment.
  • Experience: Prior experience in healthcare and the use of EHR systems; bilingual in English and Spanish/Creole highly preferred.
  • Physical Ability: Physical ability to perform functions of job (lifting, mobility, and manual dexterity).
