Key Responsibilities
  • Assess and Plan Care: Conduct thorough assessments of clients' medical needs, develop personalized care plans, and implement evidence-based interventions to promote optimal health outcomes.
  • Provide Skilled Nursing Care: Administer medications, perform wound care, vital sign checks, and other skilled nursing interventions as needed.
  • Support Client Autonomy: Encourage clients to participate in their care, provide emotional support, and promote independence in daily activities.
  • Collaborate with Interdisciplinary Team: Communicate effectively with clients, caregivers, and healthcare professionals to ensure seamless care coordination and transitions.
  • Document Care: Accurately and thoroughly document client care, including progress notes, medication administration records, and other relevant information.
Requirements
  • Active Massachusetts RN or LPN License: Must be licensed to practice as a registered nurse or licensed practical nurse in the state of Massachusetts.
  • Valid Massachusetts Driver's License: Must possess a valid driver's license and reliable transportation.
  • Experience and Skills: Preferably 1+ year of nursing experience, with a focus on geriatric care; proficiency in electronic medical record documentation; excellent communication, organizational, and interpersonal skills.
