Job Summary

We are seeking a skilled Licensed Practical Nurse to join our team at Levindale Hebrew Geriatric Center and Hospital. As a member of our nursing staff, you will provide high-quality patient care and work collaboratively with our interdisciplinary team to ensure the best possible outcomes for our residents.

Responsibilities
  • Develop and implement individualized care plans to meet the unique needs of each resident
  • Provide direct patient care, including administering medications, performing vital signs, and assisting with daily living activities
  • Collaborate with the interdisciplinary team to coordinate care and ensure seamless transitions
  • Participate in quality improvement initiatives to enhance patient care and safety
Requirements
  • Graduate of an accredited school of nursing with a current LPN license
  • Basic computer skills and experience with electronic medical records
  • Ability to work effectively in a fast-paced environment and prioritize tasks to meet deadlines
  • Strong communication and interpersonal skills, with the ability to work collaboratively with patients, families, and healthcare team members
