Peer Support Specialist/Care Coordinator

About the Position
THRIVE SS Inc is seeking a highly skilled and compassionate Peer Support Specialist/Care Coordinator to join our team. The successful candidate will possess significant experience working with vulnerable populations and in the nonprofit sector.
The Peer Support Specialist/Care Coordinator will provide structured and scheduled activities for individuals living with HIV, focusing on enhancing their natural supports, coping and self-management skills, and community engagement.
The ideal candidate will have excellent verbal and written communication skills, be highly attentive to details, and possess a high school diploma or GED equivalent.
Key Responsibilities:
- Direct, coordinate, and manage the activities of a comprehensive health program
- Ensure program compliance with HIPAA, relevant laws, and regulations
- Provide education and training to clients and others
- Develop and implement individualized care plans
- Assist clients in accessing benefits and services
Desired Qualifications:
- Significant experience working with vulnerable populations and in the nonprofit sector
- Prefer experience working on HIV-related issues or with people who experience HIV/AIDS
- Excellent verbal and written communication skills
- Highly compassionate and attentive to details
- High school diploma or GED equivalent
