Key Responsibilities:

  • Coordinate home care services and develop personalized care plans for clients
  • Communicate effectively with clients, caregivers, and healthcare professionals to ensure high-quality care
  • Manage and maintain accurate records of client information and care plans
  • Collaborate with the administrative team to ensure efficient and effective care coordination

Requirements:

  • M.A. or equivalent in a related field
  • Experience working with home care and Medicaid waiver programs
  • Excellent communication and organizational skills
  • Ability to work independently and as part of a team
