Peer Support Specialist/Care Coordinator

THRIVE SS Inc. is seeking a highly skilled and compassionate Care Coordinator for HIV Services to join our team. As a Care Coordinator, you will play a vital role in providing peer support services to individuals living with HIV.
Key Responsibilities:
- Direct, coordinate, and manage the activities of a comprehensive health program.
- Ensure the program operates in full compliance with HIPAA, relevant laws, and regulations.
- Develop and implement individualized care plans to enhance the development of natural supports, coping, and self-management skills.
- Provide supportive services to assist individuals in community engagement.
- Collaborate with clients, families, and healthcare providers to ensure seamless care coordination.
Requirements:
- Significant experience working with vulnerable populations and in the nonprofit sector.
- Prefer experience working on HIV-related issues or with people who experience HIV/AIDS.
- Excellent verbal and written communication skills.
- Highly compassionate and attentive to details.
- Ability to work evenings and weekends a few times a month and quarterly travel.
Physical Requirements:
- Frequent sitting and/or standing.
- Repetitive movement of hands and fingers - typing and/or writing.
- Visually or otherwise identify, observe, and assess.
Desired Qualifications:
- High school diploma or GED equivalent.
