Job Overview

GENERAL SUMMARY

The Case Manager is responsible for crafting and overseeing Individual Service Plans while ensuring compliance with individual goals and regulatory standards.

KEY RESPONSIBILITIES

  • Develops and manages all Individual Service Plans, including annual and semi-annual reports, utilizing HCIS format based on input from Community Resource Specialists.
  • Facilitates the scheduling and gathering of necessary documentation for Individual Service Plans and team meetings.
  • Maintains comprehensive documentation related to individual progress notes, making adjustments in line with Individual Service Plans outcomes and goal changes to support Community Resource Specialists in tracking progress.
  • Participates in all Individual Service Plans meetings and collaborates with human service agencies to assist individuals effectively.
  • Oversees the management of individual case files and documentation to ensure adherence to agency and state regulations.
  • Collaborates with other relevant departments to share objective development prior to Individual Service Plans meetings.
  • Provides assessment and orientation materials to Community Resource Specialists.
  • Monitors and evaluates participant progress through meetings with individuals and specialists, reporting findings to the CBDS Coordinator.
  • Keeps monthly schedules of program participation in community activities and services.
  • Attends all required training and informational meetings.
  • Acts as a mandated reporter for suspected cases of abuse and/or neglect to the appropriate authorities.

QUALIFICATIONS

  • Bachelor's degree required.
  • A minimum of 3 years of experience working with adults with disabilities, including assessment, community outreach, and job placement management.
  • Valid driver's license.
  • Familiarity with funding sources and relevant agencies.
  • Strong attention to detail.
