Job Title: Home Care Case Manager

At Greater Lynn Senior Services, we are seeking a compassionate and skilled Home Care Case Manager to join our team. As a Case Manager, you will work with older adults and people with disabilities to ensure they have access to the services they need to live independently.

Job Summary:

The Home Care Case Manager will conduct in-home assessments to determine the needs of our clients, educate them about available services, and assist them in securing the services they need. This is a full-time position, 40 hours per week, with a 4-day work week available.

Responsibilities:
  • Conduct in-home assessments to determine the needs of clients
  • Develop and implement person-centered care plans
  • Coordinate, authorize, and secure community-based services
  • Modify and adjust care plans as needed
  • Advocate for clients and their families
Qualifications:
  • Experience in the human services field or Bachelor's degree
  • Bilingual Russian required
  • Expert time management and computer skills
  • Strong desire to help others and establish positive relationships
  • Ability to handle confidential information
  • Reliable transportation for home visits
